To access any NHCVS Breakaway service children and young people must have a referral from the Disabled Children's Brokerage Team.

 

If you are currently receiving any support from the Disabled Children's Team please contact the Brokerage team on 01438 844254 to check eligibility.

 

Please remember that all applicants must also have registered on the SBLO online portal.

 

This portal allows you to register your interest in short break activity providers; renew your application; update your details; see the total number of hours you have remaining/used.

  

Your account will show the full annual hours you are entitled to. The SBLO year runs from 1 April to 31st March.
