How to apply 



To access any NHCVS Breakaway service children and young people must have a referral from the Disabled Children's Brokerage Team.

If you are currently receiving any support from the Disabled Children's Team please contact the Brokerage team on 01438 844254 to check eligibility.

Please remember that all applicants must also have registered on the SBLO online portal, which allows you to register your interest in short break activity providers; renew your application; update your details; see live total of the number of hours you have remaining/used (please note that totals are updated by your activity provider(s) approximately every 6 weeks).






Your account will show the full annual hours you are entitled to starting from 1 April 2018. This makes it more straightforward for providers to manage on a financial year basis.
